Disability Assistance Program

The Disability Assistance Program provides assistance to patients referred through Moses Cone Health System in filing for Social Security Disability benefits.

We assist our clients with:

  • Gathering needed medical and employment information.
  • Completing necessary paperwork.
  • Making appointments with Social Security.
  • Reconsideration if initial claims are denied. 

As time allows, the Disability Assistance Program will also provide assistance to clients referred by members of the Homeless Prevention Coalition.
