Assistance Program
The Disability Assistance Program provides
assistance to patients referred through Moses Cone Health
System in filing for Social Security Disability benefits.
We
assist our clients with:
- Gathering
needed medical and employment information.

Completing necessary paperwork.

Making appointments with Social Security.

Reconsideration if initial claims are denied.
As
time allows, the Disability Assistance Program will
also provide assistance to clients referred by members
of the Homeless Prevention Coalition. For
